Diabetes Risk Assessment

SD Diabetes CoalitionThere are 21 million Americans with diabetes - and nearly one-third of them (or 6.2 million people) don't know it! Take this test to see if you are at risk for having diabetes.

If you weigh the same or more than the amount listed for your height, you may be at risk for diabetes.

To find out if you are at risk, circle the points for each statement that is true for you. If a statement is not true, circle zero. Then add all the points to get your total score.

Scoring 0-5 points:  Low Risk
Right now your risk for having pre-diabetes or diabetes is low. Your risk goes up as you get older.  If you are Hispanic/Latino or Native American, you are at greater risk for diabetes.


Scoring 6-10 points:  At Risk for Pre-Diabetes
You are at higher risk for pre-diabetes which means your blood glucose may be higher than normal but not high enough to be diagnosed as diabetes.  Talk to your doctor about ways to reduce your risk for diabetes.


Scoring 11 or more points:  High Risk
You are at higher risk for having type 2 diabetes. Only your health care provider can determine if you have diabetes.  Talk to your doctor to see if additional testing is needed.
