Diabetes Program
The SDUIH Diabetes Program serves the needs of the Native American people with diabetes. Clinics are located in Pierre, Aberdeen and Sioux Falls, SD. Each clinic networks with other diabetes programs to pool our resources to address the needs of diabetics and their families. Our primary focus is on the youth with preventive education to eliminate diabetes. Our secondary focus is on the people who are diagnosed with diabetes. We also work with people who are on dialysis and have retinal, foot, and heart problems.

Primary Prevention Objectives
- To prevent diabetes in children and adolescents
- To prevent diabetes in family members of diabetic patients
- To prevent high-risk diabetes in high-risk adult individuals
- Target Population:
- Grades K-12
- all family members of diagnosed diabetics
- all high-risk adults showing classic diabetic symptoms.

Secondary Prevention Objectives
- To prevent complications of diabetes through Glycemic control process
- To prevent complications of diabetes through hypertension and control education
- To prevent diabetes in family members of diabetic patients
- Target Population:
- All adults and adolescents
- children with family history of diabetes
- all diabetic patients interested in counseling.

Measured Outcomes
- Incidence/prevalence of obesity, physical activity levels of children by age, nutritional intake of children by age.
- Blood pressure control
- Number of new cases in education counseling
- The data that is derived from these outcomes will be used to further enhance the diabetes program. It will become part of the medical records and will be confidential.
- Control blood glucose level by hemoglobin testing
- Exercise and frequency of target population, incidence of diabetes in target population.
- Number of patients with hemoglobin testing
- The services of support groups, peer counseling, spiritual advisors, and educators/nutritionist will be established at each site.
